2010 Registration Forms
- Please fill out the registration package and return it to the school office. Each preschool and Kindergarten registration package must include a health form from your doctor's office. Your pediatrician should have these forms on file or you may download ours.
To Fax Forms: Save the registration package file to My Documents or similar folder of your choice on your computer, fill it out, print it, and fax it to 703-591-1431
To Email Forms: Save the registration package file to My Documents or similar folder of your choice on your computer, fill it out, and attach file to registration@merrittacademy.org
